Dr Hoffman qualified as a Pediatrician in 1980 and thereafter he held an academic position for eight years at Tygerberg Hospital, 2 Military Hospital and the Medical School of the University of Stellenbosch. During this time he lectured, did research and published in international journals.
He spent the next eight years in private practice at the Panorama MediClinic. During this time he established a neonatal intensive care unit that is still today one of the best units in South Africa. During these years he played an important role in the Medical Association as Member of the Board of Trustees, member of the Executive Committee and Chairman of the Private Practice Committee.
In 1996 he joined Sanlam Health as Medical Director and General Manager of the Managed Health Care Division. He was responsible for the implementation of managed health care in Sanlam Health after a period of study in the USA. During this time he worked closely with Capp Care (California) and US Healthcare (Philadelphia).
He was appointed as director of the Council for Health Care Accreditation of Southern Africa and the Interim Council of the Representative Association of Medical Schemes. He was the founder of African Health Synergies, a preferred provider network development organisation. Dr Hoffman is still seen as one of the foremost experts in the field of managed healthcare in South Africa.
Dr Hoffman left Sanlam Health at the end of 1999 and was recruited by Africa Group to help with the establishment of Multimed, a Medical Scheme Administrator. He spent 2 years as a director of Multimed and at the same time consulted for Medscheme Integrated Care.
He was appointed as CEO of Multimed in January 2002 and resigned from Africa Group at the same time. The purpose of his appointment at Multimed was to restructure the company in order to increase the efficiency and competitiveness of Multimed.
He started the business transformation process in Multimed in early 2002 and in 2003 the National Productivity Institute awarded the Golden Award to Multimed. Multimed was only the second Medical Scheme Administrator that was ISO 9001/2000 certified and also achieved the position as the best Medical Scheme Administrator in 17 of the 19 categories in the 2004 Twigg survey. He was instrumental in the design and development of a Managed Health Care capability for Multimed, which was implemented in 2004. During 2005 and 2006 he conceived and designed a Disease Management Module for the management of chronic diseases (CDL or Chronic Disease List).
In 2005 the Professional Provident Society (PPS) decided to re-enter the medical scheme administration business in order to administer Profmed. Dr Hoffman proposed that a business replication model of Multimed be implemented. Within 10 weeks after the contracts were signed, PPS Medical Scheme Administrators (PMSA) started functioning as an independent administration company. PMSA was ISO 9001/200 certified by the SABS in 2006.
During 2005, African Life Health acquired Multimed. Within 6 months Momentum acquired African Life Health and Multimed became part of Momentum Medical Scheme Administrators (MMSA). Dr Hoffman was appointed as Head of Administration of MMSA. During 2007 he was responsible for the merger of 4 administrators, Multimed, Ingwemed, Amanzi and Impilo into Multimed, and in 2008 the merger between Multimed and Sovereign Health to form a single administration company, MMSA. MMSA had 11 medical schemes under administration with a total of 200 000 principal members.
Dr Hoffman resigned at the end of 2008 and took up the position as CEO of PPS Healthcare Administrators in Centurion, with the aim of growing the business to a multi-scheme administrator.
Since 2009, PPSHA has grown from a single scheme administrator to the 5th largest in the industry. During this time membership under administration has grown from 24 000 to 82 000 principal members. The annual premium income increased from R0,6 billion to R 3.0 billion. Administration fee income rose from R45 million to R200 million and profit before tax increased tenfold from R3,5 million to R35 million.
Since PPSHA took over the administration of Profmed and Keyhealth they experienced profound improvements in their statutory reserve levels. This was achieved largely due to a PMSA driven planning process that saw refined changes to benefit designs and reductions in claims ratios through financial and clinical risk management principles. Both schemes are now financially sound and growing in membership.
Dr Hoffman retired at the end of March 2018 and is currently an independent consultant for various health care entities.
